Please do not use this form if you have an urgent medical problem or you need to reschedule an existing appointment. Instead, contact the office you were planning to visit. After you submit your request, our appointment scheduler will respond within 48 hours. See our appointment policy below. You can also download new patient forms to save time in the office.

If you are an established patient, we recommend requesting appointments through our online patient portal to provide you with faster service. Creating an account is simple and enables you to do all these things online at your convenience:

Appointment Policy

Colonial Healthcare understands that sometimes people suddenly become ill and need to see a physician the same day. For that reason, our providers make time to see patients without appointments.

Appointments are preferred to keep patient wait times to a minimum. Due of the number of patients we see, it is important to keep your appointment or cancel at least 24 hours before. There is a fee for no-show appointments, as there are costs associated with scheduling for our practice.

Colonial Healthcare does make phone call reminders 24 hours prior to your visit. Please make sure we have your correct number on file.
